An analysis of human reliability during normal and abnormal operations, including mitigating and initiating actions, is required by OSHA 1910 "Process Safety Management", Process Hazards Analysis, and DOE 5480.23 "Safety Analysis Review", Chapter 5 and Chapter 13. This analysis is accomplished by using human factors checklists to evaluate performance shaping factors. This performance shaping factor analysis is used in a human reliability analysis to evaluate the probability for human action during normal and abnormal operations. This support also includes an analysis of the consequences of the human actions and means for improving the probability the intended actions are taken.
